ITEMS YOU WILL NEED TO ENROLL

YOUR NEW STUDENT INTO

CHARLOTTE MIDDLE SCHOOL

 

(Note:  Are you the child’s legal guardian?  If not, you must register at the School Board Office)

The following requirements are for students who are new to Dickson County:

  • Three (3) proofs of residence within the CMS school zone.  (examples:  Utility bills, paycheck stub, copy of driver’s license, voter registration, mortgage, deed or letter from landlord)
  • Official state birth certificate
  • Current immunization records on a Tennessee Immunization form
  • Completed registration form (forms are available online at http://www.dicksoncountyschools.org/parents/forms_documents/ or in the school office

**       If you child is entering from out-of-state or registering for the first time in Tennessee public schools, a current physical is required.

***     If you are transferring from another Dickson County school, you only need to provide proof of residence and complete a registration form.


 CMS welcomes Dr. Justin Barden as our new principal. Dr. Barden taught and served as an Assistant Principal in Humboldt (West Tennessee) for nine years. He moved to Dickson County and has served as an Assistant Principal at DMS for the past three years. 

Dr. Barden is an undergrad graduate from Union University. He also received his Masters from Trevecca University, before earning his Ed.S and Ed.D from Union University. 

Dr. Barden has been married for 13 years to Allyson, and has four beautiful children: Baylee, Kynlee, Jackson, and Deacon. He and his family also attend First Baptist of Dickson. 

Barden
